Older Person Hospital Discharge service

Hestia's Older Person Hospital Discharge service is a short-term service for Ealing residents aged 55 and over.

Our team ensures that service users leaving hospital are able to return to a secure, warm and clean home environment. Service users are then supported to ensure appropriate services are in place to aid their recovery and help them get back to independent living.

If further support is needed once discharge-related assistance has been completed, the service will internally refer individuals to Hestia’s Ealing Older People Floating Support service. This service will then continue working to help people maintain independence, healthy and active lifestyles, and reduce social isolation.

This service only accepts service users via referral from hospital discharge coordinators, social workers and safeguarding teams.
