Hounslow LIFE: Integrated Support Service
Putting you in control and supporting you to live independently
Hestia’s new Hounslow LIFE Integrated Support Service (ISS) is part of a significant modernisation project led by the London Borough of Hounslow with the support of NHS Hounslow CCG.
The new service covers 3 elements:
- Floating support (all client groups)
- Care Act advocacy support
- Carers short breaks (Hestia is sub-contracting this to our partners Carers Trust Thames)
The service will support almost all groups of vulnerable adults in the community aged 16+. This includes people:
- with a learning disability
- who are socially excluded
- with autism
- with mental health needs
- with physical and sensory conditions
- with long term conditions
- young people
- older people

Our team are located in the 5 key localities in Hounslow, making it easy for them reach as many services users efficiently and forge effective partnerships to deliver holistic recovery based support. The service model focuses on preventing needs from escalating while supporting service users to identify their needs and strengths and to self-manage their risks as well accessing community based support. Our team takes a whole household approach to identify needs wider than the direct service user, and so promote independent living for everyone we engage with.
The service will be flexibly delivered Monday – Friday 8-9pm and weekends (plus bank holidays) 9-5pm (slight variations for each element).
*NB: if you used to access support through our mental health floating support team your support will not change. However it will now be delivered via the Hounslow LIFE ISS team.
